Candidate application of a party


The following shall be appended to the candidate application of a party:


1)  a proposal for the list of candidates of the party, stating in the order in which the candidates are placed in the master list of candidates, the name and title, profession or occupation of each candidate, using no more than two expressions, and their municipality of residence; along withor instead the given name, a generally known name of the person or his or her abbreviated first name may be used; the list of candidates must not include any other entries, unless they are necessary to clarify the identity of the candidate;


2)  a signed document from each candidate including the candidate's personal identity code and stating that he or she consents to stand as a candidate for the party that has nominated him or her and that he or she consents to assume the responsibilities of a Member of the European Parliament, as well as containing an affirmation that the candidate has not agreed to stand as a candidate for another party or a constituency association or as a candidate in the sameelections;


3)  an affirmation where each candidate who is a citizen of another Member State of the European Union confirms that he or she has not agreed to stand as a candidate in another Member State in the same elections or lost his or her candidate eligibility in the elections to the European Parliament in his or her home state, as well as a notification stating the citizenship of the candidate, the electoral district or municipality in the candidate's home state in which he or she most recently was entered in the electoral roll for the elections to the European Parliament, and his or her most recent address in the home state.


The polling representative of a party shall date and sign the candidate application and affirm that the candidates are eligible.
